• Membership card that is not expired

EMT/EMS Requirements:

• Current ID Card that includes their name and title

911 Dispatcher Requirements:

Must provide one of the following:

• Recent training completion certificate

• Copy of a valid, professionally-issued certification illustrating 911 Dispatcher status

Healthcare Professionals Requirements:

• Must provide copy of a valid state-issued license
